Instructions for using the ESE Forms.

1. All forms are listed in numerical order.  To access a particular form, click on the number group your form is located.  This is located under the picture of the man working at the computer.

2. Once you have clicked on the group link,  you will see a list of forms.  Click on the form number/name you need.

3. Fill in the information on the form as needed.

4. You can then print the form and it will have the information you entered.  Click File—Print

5. If you desire to save the modified form to your documents click “Back” on the browser toolbar.  You will be given the options to: “Yes—Save your changes”, “No—Discard your changes”,  or “Cancel—Keep this document open”.  Make your desired choice then you will return to the documents choice list.
